INVITATION FOR TECHNICAL AND FINANCIAL BIDS FOR THE EXECUTION OF NHIS INTERVENTION PROJECTS: UDUTH/ADM/1720
The National Health Insurance Scheme (NHIS) has allocated funds for Intervention Projects in Usman Danfodiyo University Teaching Hospital, (UDUTH) Sokoto towards achieving the Hospital’s mandate.
The Usman Danfodiyo University Teaching Hospital, Sokoto therefore wishes to invite reputable contractors with relevant proven competence and experience to submit tenders for the execution of the following projects:
2.0 PROJECT DESCRIPTION
DESCRIPTION OF FACULTY: Procurement, Installation and Commissioning of 2 Nos. 60KVA Power Generating Sets for NHIS Clinic and Oxygen Plant in UDUTH Sokoto
LOCATION: UDUTH Sokoto
DESCRIPTION OF FACULTY: Procurement, Installation and Commissioning of Medical Equipment in some specialty areas in the Hospital
LOCATION: UDUTH Sokoto
3.0 SCOPE OF THE PROJECT
The Scope of the Projects are detailed in the bidding documents which shall be issued to prospective bidders.
4.0 ELIGIBILITY REQUIREMENTS
Prospective bidders must possess the under-listed requirements to be eligible to participate in the procurement:
i. Certificate of Incorporation with the Corporate Affairs Commission (CAC)
ii. Evidence of Company’s Current Tax Clearance Certificate
iii. Copies of Audited Account for the last three (3) years (2014, 2015 & 2016)
iv. Evidence of Current Pension Clearance Certificate in compliance with the Amended PenCom Reform Act 2014
v. Evidence of Registration on National Database of Contractors, Consultants and Service Providers by inclusion of Interim Registration Report (IRR) issued by the Bureau of Public Procurement (BPP)
vi. Evidence of Current ITF Certificate in compliance with Industrial Training Fund (ITF) Act, 2005
vii. Verifiable evidence of financial capability with letter of commitment from Bank
viii. Current sworn affidavit disclosing:
a. That the company is not in receivership, insolvent or bankrupt
b. That the company does not have any Director who has ever been convicted in any court in Nigeria or any other country for criminal offence in relation to fraud or financial impropriety
c. Whether or not any officer of the Usman Danfodiyo University Teaching Hospital, Sokoto is a former or present Director, Shareholder or has any pecuniary interest in the bidder
d. That all documents submitted for the bidding are true and correct in all particulars
ix. List of verifiable documentary evidences of at least two (2) similar jobs successfully executed within the last three (3) years including letters of awards and job completion certificates
x. List of key technical personnel indicating their roles, designation, qualification (enclosed copies) and years of experience with the guarantee that a minimum of three (3) relevant registered professionals shall be deployed on the project. Copies of professional and educational certificates should be provided
5.0 COLLECTION AND SUBMISSION OF BID DOCUMENTS
Interested and competent bidders are requested to collect bid documents from the Office of the Deputy Director Procurement, Administration Block, Usman Danfodiyo University Teaching Hospital, Sokoto upon presentation of evidence of payment of non-refundable tender fee of N10, 000.00 (Ten Thousand Naira) only for each Lot.
Prospective bidders shall enclose their Technical document in separate envelope (marked Technical) and the Financial Bid in another envelope (marked Financial) and the two separate envelopes shall be enclosed in one sealed outer envelope clearly marked “INVITATION TO TENDER –Lot NO. (Indicate the Lot the bid is for) in three (3) sets: one (1) original and two (2) copies. Submissions should be addressed to:

The completed and sealed bid documents should contain the bidder’s name on the reverse side of the envelope and be deposited in Tenders Box in the Office of the Deputy Director Procurement, Administration Block, Usman Danfodiyo University Teaching Hospital, Sokoto Usman Danfodiyo Usman Danfodiyo University Teaching Hospital, Sokoto between the hours 8:00am – 4:00pm Monday to Friday.
6.0 CLOSING DATE AND OPENING
Submission of bid documents closes on Tuesday 28th November, 2017 at 12:00 noon. A bid received after the stated date and time shall not be considered. The submitted Technical bids will be opened immediately after the closing in the Board Room, Administration Department UDUTH Sokoto. Successful bidder(s) would be invited for the opening of their Financial Bids at a later date after the Technical Evaluation. The Financial Bids for unsuccessful bidders would be returned to them unopened. Bidders/Company representatives and interested members of the public including relevant professional bodies, Civil Society and Non-Governmental Organizations are hereby invited to witness the public opening of the bids.
7.0 PLEASE NOTE THAT
i. Certificates may be referred to the respective issuing authority for verification and authentication.
ii. Technical and Financial Bids enclosed in one and same envelope that will expose the Technical and Financial bids at the same time of opening shall be disqualified.
iii. Submissions that do not meet up with the requirements specified above shall be rejected.
iv. The Evaluation of bids shall be strictly based on the requirements specified above.
v. This advertisement is published for information purpose, and should not be construed as a commitment or obligation on the part of Usman Danfodiyo University Teaching Hospital to award these contracts.
vi. The Usman Danfodiyo University Teaching Hospital, Sokoto is not bound to consider any bid and reserves the right to reject all bids at any time prior to the acceptance of a bid or cancel the procurement proceeding in the public interest.
