ADDENDUM TO OUR PREVIOUS ADVERT IN THE FEDERAL TENDERS JOURNAL OF MONDAY 5TH FEB TO SUNDAY FEB 18TH 2018 AND NEWSPAPER ADVERTS OF TUESDAY, FEBRUARY 13, 2018 BOTH IN DAILY TRUST AND THE GUARDIAN FOR THE IMPLEMENTATION OF 2017 CAPITAL PROJECTS
In line with the guideline of the Bureau of Public Procurement, University of Nigeria Teaching Hospital Ituku-Ozalla Enugu, wishes to invite credible, competent and registered contractors to bid for Non-Consultancy Services for the Medical outreach at Awgu Federal Constituency, Enugu State.
2.0 SCOPE OF WORK
Medical Outreach for Awgu Federal Constituency for about 1 week
Use the under-listed cost components to submit your proposal
a. Cost of Drugs
b. Staff Remuneration
c. Reimbursable Expenses
3.0 MANDATORY REQUIREMENTS
i. Verifiable evidence of Certificate of registration with Corporate Affairs Commission (CAC) by inclusion of Certificate of Incorporation.
ii. Company’s Tax Clearance Certificate for the last three consecutive years.
iii. Evidence of VAT Registration and up-to-date remittance.
iv. Evidence of compliance with Pension Reform Act, 2004 by inclusion of Compliance Certificate from National Pension Commission (PENCOM).
v. Verifiable evidence of certificate of compliance by Industrial Training Fund in line with ITF Act 2011.
vi. Verifiable evidence of compliance with the Employee Compensation Act which requires all employer of labour to remit 1% of the total annual emolument of workers to National Social Insurance Trust Fund (NSITF).
4.0 ADDITIONAL REQUIREMENTS
i. Evidence of current license with the Nigeria Medical Association must be included by medical personals involved.
ii. Detailed company profile, showing verifiable evidence of professional experience in a similar job, by submission of at least three projects of similar nature and complexity applied for, with letters of award and job completion certificate in the last three years.
iii. Sworn affidavit indicating the following:
a. That the company is not in receivership/insolvency/bankruptcy
b. That no officer of the procuring entity (University of Nigerian Teaching Hospital, Enugu), is a former or present Director, shareholder or has any pecuniary interest in the company.
c. Not having any Director who has been convicted in any country for criminal offense.
d. Confirming that the bid and all other information submitted are true.
5.0 ADDITIONAL REQUIREMENTS
Interested bidders are to pay a non-refundable tender fee of Twenty Thousand Naira (N20,000.00) only into the Treasury Single Account (TSA) of the University of Nigeria Teaching Hospital (UNTH), Enugu. The receipt will enable bidders collect the bidding documents at the office of the Head, Procurement.
6.0 SUBMISSION OF BID
Interested bidders are requested to submit their bid in a sealed big brown envelope clearly marked “Expression of Interest for Non-Consultancy Services for Medical Outreach at Awgu Federal Constituency, Enugu State” and addressed to the:

7.0 CLOSING OF BIDS SUBMISSION
Closing date for all submissions should be on or before 27th March, 2018.
8.0 OPENING TIME OF EXPRESSION OF INTEREST
i. Opening of the received documents will commence immediately after the closing date at 12:00pm prompt at the Boardroom, UNTH, Ituku-Ozalla, Enugu.
ii. Only the bidders that meet the qualification criteria shall be invited for interview.
iii. The procuring entity reserves the right to reject any submission that does not meet the requirement as specified.
9.0 OTHER INFORMATION
Any mutilated bid will not be accepted. Further inquiries should be directed to office of the Head of Procurement Department.
This advertisement is published for information purpose only and does not guarantee an award of contract. A contract shall be awarded to the lowest evaluated responsive bid from the bidders substantially responsive to the bid solicitation (see section 16(17) and 24(3) of PPA, 2007. The burden of proven fulfillment of the requirements for participation in the EOI solicitation lies on the contactor/supplier (see 16(16) of PPA, 2007), hence, the Hospital is not to be held liable to refund any cost/expenses incurred by interested Party(ies) in connection with the invitation.
