INVITATION TO TENDER FOR THE CONSTRUCTION OF PROPOSED NHIS BUILDING FOR THE UNIVERSITY OF CALABAR TEACHING HOSPITAL, CALABAR
In pursuance to the execution of intervention Projects, The University of Calabar Teaching Hospital, Calabar hereby invites competent and reputable contractors to submit bids for the construction of proposed NHIS building for the University of Calabar Teaching Hospital, Calabar.
Interested and reputable companies are hereby invites to submit Technical and Financial Bids accordingly.
i. Construction of Proposed NHIS Building for University of Calabar Teaching Hospital, Calabar.
2. TECHNICAL REQUIREMENTS
Interested bidders are required to submit the following along with their bids:
i. Evidence of Registration with Corporate Affairs Commission (CAC).
ii. Evidence of Company’s Current Tax Clearance Certificate for the last three (3) years – 2015, 2016 and 2017.
iii. Company’s Audited Accounts for the last three (3) consecutive years (2015, 2016 and 2017) corresponding with declared Annual Turnover and Profit of the Audited Account.
iv. Evidence of Valid Compliance (Certificate) with the provision of Section 6(1) (3) of the amended Industrial Training Fund (ITF) Act no.19 of 2011.
v. Evidence of Valid current PENCOM Certificate in compliance with the Pension Reform Act.
vi. Evidence of Compliance Certificate with the Nigeria Social Insurance Trust Fund (NSITF).
vii. Evidence of current Interim Registration Report (IRR) as evidence of registration with the Bureau of Public Procurement (BPP) in line with provisions of Section 5(h) of the PPA 2007.
viii. Evidence of Financial Capability from a reputable Bank.
ix. Verifiable evidence of (3) similar jobs successfully executed or being executed in Nigeria over the last 5 years including letters of Award of contracts and job completion certificate with at least One Hundred Million (N100,000,000) value in a single contract per year for five (5) years. (Please provide contact addressed of the clients who may be contacted for reference).
x. Verifiable Financial Statement indicating availability of funds of not less than (N50,000,000) in the Company’s Account in the last three (3) years.
xi. Comprehensive Company Profile with convincing proof of Technical competence to undertake the jobs within the required time frame.
xii. List of verifiable equipment indicating ownership or on lease (for construction works only).
xiii. Evidence of CV’s of technical, operational and managerial staff with relevant work experience in regards of the scope of work.
xiv. A sworn affidavit confirming that none of the Directors has been convicted in any court of any criminal offence. In addition that the company is not bankrupt or insolvent.
xv. A sworn affidavit confirming that all information presented and attached are true and correct of the company and that no officer of University of Calabar Teaching Hospital, Calabar or BPP is a former or present Director, Shareholder or has any pecuniary interest in the bidder.
3. COLLECTION OF TENDER DOCUMENTS
Interested companies can obtain the Tender Document from the Procurement Unit, University of Calabar Teaching Hospital, Calabar upon payment of non-refundable fee of N10,000.00 (Ten Thousand Naira) only.

3. Make payment using the preferred option.
4. SUBMISSION OF BIDS
a) Technical Bids should be in three copies and should be in a sealed envelopes labeled “Technical” clearly marked, with the project title at the right hand corner of the envelope and addressed to:

b) The documents should be deposited in the tender box at the Office of Head of Procurement Unit, University of Calabar Teaching Hospital, Calabar. Company names should also be written at the reverse side of the envelope.
Tender submission will close by 1:00pm on Monday, 12th March, 2018.
Incorrectly labeled and late submission of tenders will be rejected.
Opening of bid shall commence immediately following the submission deadline at 1:00pm on Monday, 12th March, 2018 in the Board Room, University of Calabar Teaching Hospital, Calabar. All bidders and/or their representatives, Civil Rights organizations, relevant Professional Bodies and NGOs are hereby invited to witness the public Opening exercise accordingly.
6. GENERAL INFORMATION
a) All CAC, PENCOM, ITF, FIRS, BPP Certificates and/or letters may be referred to the Security Agencies, Corporate Affairs Commission (CAC), Federal Inland Revenue Service (FIRS), National Pension Commission, Bureau of Public Procurement (BPP) and other relevant bodies for verification.
b) The University of Calabar Teaching Hospital, Calabar (UCTH) shall verify the authenticity of evidence submitted by bidders with the relevant authorities. Any company found to have submitted forged documents will be handed over to the relevant agencies for possible prosecution.
c) All documents listed above must be arranged in the order of listing with Page numbered and Signed accordingly.
d) Original documents of any aspect of a company’s submission will be requested for sighting before award of contract.
e) Only pre-qualified bidders will be invited for the next stage of procurement process.
f) All Companies Letter Head Paper must have contact telephone numbers, valid e-mail address as well as the Names and Nationalities of the Directors.
g) The University of Calabar Teaching Hospital, Calabar reserves the right to reject any or all the submission that do not meet up with the requirements specified.
h) Prospective contractors should ensure that they sign the register provided on submission of bid documents.
The UCTH is by this Advert not bound to award contract to the lowest Bidder.
Nothing in this advert shall be construed to be a commitment by the University of Calabar Teaching Hospital, Calabar nor shall it entitle any contractor to make any claim(s) and/or seek any indemnity from the University of Calabar Teaching Hospital, Calabar
All expenses incurred in preparing bid documents in response to this advertisement is borne by the prospective contractor.
