INVITATION TO TENDER FOR SUV V6 4.0 LITRES ENGINE 2019 MODEL, 2.5 LITRES PETROL ENGINE 2019 MODEL AND 1.8 LITRES PETROL ENGINE 2019 MODEL
1.1 ISTH is a 369 bedded hospital and a Centre for Excellence in medicine particularly, in the areas of Rural and Sub-Urban medicine and the diagnosis and Management of viral hemorrhagic fevers especially Lassa Fever. The vehicles for purchase in this tender are Chief Medical Director’s official vehicle and operational vehicle as most of the operational vehicles procures in 2013 are old with high maintenance cost. There is also a need for new ones due to expansion.
1.2 Tenders are hereby invited from suitably qualified and experienced Contractors/Suppliers to submit bids for the under-listed projects:
- SCOPE OF WORK/SUPPLY
2.1 LOT I: Supply of I No. 2019 SUV VX-L (V6) 4.0 Litres Petrol Engine.
Type: Keyless Go (Plus Button Start), Anti-Theft immobilizer with alarm, Anti-lock Braking system (ABS) cool Box, Automatic Dual Air Conditioning System, front Fog Lamps, 8 Air Bag System Leather seats, Aluminum Alloy Wheel, AM/FM Radio/CD Player, Power Seat lock and Windows. Packing sensors + reverse Camera.
LOT II: Supply of 2.5L Petrol Engine
Type: Keyless Entry, 6-Speed Automatic transmission, USB Port, TRC Traction Control, Touch Screen on the dash board, 3 Zones Air Conditioning System, 10 Speakers, Anti-lock Braking System (ABS) 9 Air Bag, Leather Seats, Aluminum Alloy Wheel, AM/FM Radio/CD Player, Power seat lock and Windows.
LOT III: Supply of 2019 Model 1.8 Litre Petrol Engine
Type: Four Cylinder 140 Horse power, LE ECO Editor, Automatic, Transmission Aluminum Alloy Wheel, 6 Air Bag, AM/FM Radio/CD Player, Power seat lock and Windows.
- ELIGIBILITY REQUIREMENTS
i. Evidence of Certificate of Incorporation with Corporate Affairs Commission (CAC) including form CAC2 and CAC7;
ii. Evidence of Company’s Income Tax Clearance Certificate for the last three (3) years valid till 31st December, 2018 with minimum average turnover of (N50,000,000.00) Fifty Million Naira.
iii. Evidence of current Pension Compliance Certificate valid till 31st December, 2018.
iv. Evidence of current Industrial Training Fund (ITF) Compliance Certificate valid till 31st December, 2018.
v. Evidence of current Nigeria Social Insurance Trust Fund (NSITF) Compliance Certificate valid till 31st December, 2018.
vi. Evidence of Registration on the National Database of Federal Contractors, Consultants and Service Providers by submission of Interim Registration Report (IRR) expiring 1/1/2019 or valid Certificate issued by BPP.
vii. Sworn Affidavit disclosing whether or not any officer of the relevant committees of Irrua Specialist Teaching Hospital or Bureau of Public Procurement is a former or present Director, Shareholder or has any pecuniary interest in the bidder and to confirm that all information presented in its bid are true and correct in all particulars.
viii. Company’s Audited Accounts for the last three (3) years 2015, 2016 & 2017.
ix. Evidence of financial capability to execute the project including Reference Letter from a reputable commercial bank indicating willingness to provide credit facility for the execution of the project when needed.
x. Verifiable documentary evidence of at least three (3) similar jobs executed in the last five (5) years including, letters of award, valuation certificate, job completion certificates and photographs of the projects.
xi. Only companies included in the National Automotive Design and Development Council’s approved list of local manufacturers of vehicles or their authorized representatives should bid for the procurement.
xii. All documents for submission must be transmitted with a covering/forwarding letter under the Company/Firm’s Letter Head Paper bearing amongst others, the Registration Number (RC) as issued by the Corporate Affairs Commission (CAC), Contact Address, Telephone Numbers (preferably GSM No.), and e-mail address. The Letterhead Paper must bear the Names and Nationalities of the Directors of the Company at the bottom of the page, duly signed by the authorized officer of the Firm.
- SUBMISSION OF TENDER DOCUMENTS
Prospective bidders are to submit bid in two (2) hard copies each of the technical and financial bids with softcopy of financial bid only in MS Excel format, packaged separately in sealed envelopes and clearly marked as “Technical Bid” or “Financial Bid”. Thereafter, put the two sealed envelopes together in a bigger sealed envelope addressed to. (the Chief Medical Director at the first floor of the Hospital Administrative Building) and clearly marked with (i) Supply of I No. 2019 SUV X-L (V6) 4.0 Litres Engine (M.E SPEC) (ii) Supply of 2.5L Petrol Engine (iii) Supply of 2019 Model 1.8 Litre Petrol Engine.
Furthermore, the reverse side of each sealed envelope should have the name and address of the bidder and dropped in the designated Tender Box in the office of the (Chief Medical Director) not later than 12:00 noon on Monday 25th March, 2019.
- OPENING OF BID
The technical bids will be opened immediately after the deadline for submission at 12:00 noon on Monday, 25th March, 2019 in the Board Room of the Hospital in the presence of bidders or their representatives, while the financial bids will be kept unopened. Please ensure that you sign the Bid Submission Register in the office of Chief Medical Director, as the Irrua Specialist Teaching Hospital will not be held liable for misplaced or wrongly submitted bids. - GENERAL INFORMATION
a) Bid must be in English language and signed by an official authorized by the bidder.
b) Bids submitted after the deadline for submission would be returned un-opened.
c) All costs will be borne by the bidders.
d) Only pre-qualified bidders at technical evaluation will be contacted at a later date for financial bid opening, while financial bids of unsuccessful bids will be returned un-opened.
e) The Irrua Specialist Teaching Hospital is not bound to pre-qualify any bidder and reserves the right to annul the procurement process at any time without incurring any liabilities in accordance with Section 28 of the Public Procurement Act 2007. Signed:
