INVITATION TO TENDER FOR THE EXECUTION OF 2018 CAPITAL PROJECTS
The Federal Government of Nigeria has allocated funds to the Federal Ministry of Health in the Year 2018 Appropriation. The Ministry intends to use its budgetary allocation, Intervention funds and funds from other sources for the procurement of Goods, Works and Services/Consultancy for the execution of Policies, Programmes and Projects towards achieving its mandate.
Accordingly, the Federal Ministry of Health invites qualified, interested and eligible vendors; suppliers; contractors; Original Equipment Manufacturers (OEMs) and consultants to tender for the procurement of Goods, Works and Services appropriated under various Department of the Ministry as specified as follows:
Procurement of Stationeries, Office Equipment, Furniture, Drugs, Reagents and Commodities, HIV/Test Kits, IRS Insecticide, Net Hanger Kits, Dental Chairs, Automated Blood Screening Machine, Quality Control Equipment, Vehicles, Printing and Production of Document, Journal etc.
Category A: Goods
1. Lot A1: Procurement of Office Consumables, Furniture and Equipment
2. Lot A2: Procurement of Vehicles
3. Lot A3: Procurement of Drugs
4. Lot A4: Procurement of Medical Consumables
5. Lot A5: Procurement of Security Gadget
Procurement and Installations of various Medical Equipment; Procurement and Installation of Solar Power System at various Federal Hospitals; Maintenance and Rehabilitation Ministry Headquarters, Construction of Medical Centres, Federal Medical Centres, Outreach Centres, Occupational Health Centres, etc.; ICT Portal Development; Repair and Equipping of A & E Department at various University Teaching Hospitals, Federal Medical Centres; Constituency projects under works. These items fall into the following Lots.
Category B: WORKS
1. Lot B1: Procurement and Installation of Medical Equipment (Note: Only contractors with Authorization Letters from the BPP approved list of Manufacturers and Equipment for Tertiary Hospitals will be considered).
2. Lot B2: Construction and Equipping of Child Maternal Health Centre at Uturu, Abia State
3. Lot B3: Rehabilitation and Provision of Hospital Equipment for Accident & Emergency Unit for New Bussa Hospital, Borgu LGA.
4. Lot B4: Provision of Internal Road and Drainages in D’Kudu General Hospital and total rehabilitation of D’Kudu General Hospital Buildings
5. Lot B5: Completion of Research Laboratory Road Network Parking Lots in Lamingo Jos
6. Lot B6: Rehabilitation/Repair of Federal Ministry of Health Headquarters Building
7. Lot B7: Procurement and Installation of Solar Power System at various Federal Hospitals
8. Lot B8: Construction of Model Occupational Health Centre in Abuja
C. REQUEST FOR PROPOSAL /CONSULTANCY SERVICES
Consultancy for the Supervision of Construction/Rehabilitation projects; Conduct of Baseline Survey, Specific Researches/Investigation etc. for various Department and Agencies within the Ministry. These items fall into the following lots;
Category C: CONSULTANCY SERVICES
1. Lot C1: Consultancy for the Supervision of Construction
Category D: REQUEST FOR PROPOSAL (NON-CONSULTANCY SERVICES)
1. Lot D1: Cleaning and Fumigation Services (Bidders must present Certificate of Registration as Environmental Health Service Provider)
• Details and specification for categories of various lots are as contained in the Bid documents.
• Bidders are advised to contact the Procurement Department for further information on the categorization as sub-lots may apply to some lots for effective procurement of health commodities and equipment according to various specifications.
2. ELIGIBILITY REQUIREMENT AND QUALIFICATION CRITERIA
In order to be considered, interested bidders shall present the following documents in their technical tender;
a) Evidence of Certificate of Incorporation with Corporate Affairs Commission (CAC) including forms CAC2 and CAC7.
b) Evidence of current Pension Compliance Certificate valid till 31st December, 2018.
c) Evidence of Company Income’s Tax Clearance Certificate for the last three (3) years valid till 31st December, 2018.
d) Evidence of current Industrial Training Fund (ITF) Compliance Certificate valid till 31st December, 2018.
e) Certificate of Compliance from Nigeria Social Insurance Trust Fund (NSITF) valid till 31st December, 2018.
f) Evidence of Registration of the National Database of Federal Contractors, Consultants and Service Providers by submission of Interim Registration Report (IRR) expiring 1/1/2019 or valid Certificate issued by BPP.
g) Evidence of registration with relevant Regulation Agencies.
h) Evidence of financial capacity to execute the project including Reference Letter from a reputable commercial bank indicating willingness to provide credit facility for the execution of the project when needed.
i) Similar jobs executed in the last five (5) years including letters of awards, valuation certificate, job completion certificate and photographs of projects.
j) Company’s Audited Accounts for the last three (3) years (i.e. 2015, 2016 & 2017).
k) Evidence of Manufacturer’s Authorization of dealership/distributorship appointment where applicable (Note that only authorization letter from BPP approved list of Manufacturer and Equipment for Tertiary Hospitals will be considered).
l) Sworn Affidavit that (a) firm is not bankrupt (b) None of the Director is an ex-convict or bankrupt (c) that any officer of FMOH or Bureau of Public Procurement is a former or present Director, shareholder or has any pecuniary interest in its bid, that all information presented in its bids are true and correct in all particular.
m) Company profile with the curriculum vitae of key staff to be deployed for the project including copies of their Academic/Professional qualifications and registration with professional bodies.
n) List of Plants/Equipment with proof of ownership/lease (where applicable).
o) Bidders for cleaning and fumigation service must present registration certificate of Registration as Environmental Health Service Provider.
p) All documents for submission must be submitted with a covering/forwarding letter under the Company/Firm’s Letter Head Paper bearing amongst others, the Registration Number (RC) as issued by the Corporate Affairs Commission (CAC), Contact Address, Telephone Numbers (preferably GSM No.), and e-mail address. The Letterhead Paper must bear the Names and Nationalities of the Directors of the Company at the bottom of the page, duly signed by the authorized officer of the Firm.
• All pages of document submitted must be authenticated by the Company CEO or his/her representative, Failure will lead to disqualification.
3. COLLECTION OF TENDER DOCUMENTS
Interested bidders are to collect Standard Bidding Document from the Office of the Chief Procurement Officers at Room 302/306 3rd Floor, Federal Ministry of Health Headquarters, Federal Secretariat Complex, Phase III CBD, with their Compact Disc (CD) to collect the soft copy of the Standard Bidding Document. bidders for works are to submit only their technical documents as only technical qualified bidders will be invited to collect tender documents. Other document(s) to support the qualification and tender documents as may be contained in the Standard Bidding Documents. 4. SUBMISSION OF TENDER DOCUMENTS
Prospective bidders for goods are to submit: (A) Two (2) hard copies each of the technical and financial bids. (B) Soft copy of the financial bid for each of the Lot desired, packaged separately in sealed envelopes and clearly marked as “Technical Bid” or “Financial Bid”.
Thereafter, put the two sealed envelopes together in a bigger sealed envelope addressed to the Permanent Secretary, Federal Ministry of Health and clearly marked with (the name of the project and the Lot No.). furthermore, the reverse of each sealed envelope should have the name and address of the bidder, project title and Lot No. and dropped in the designated Tender Box not later than 12:00 noon 12th September, 2018 at:

4. CLOSING DATE AND TIME
Closing date is 12:00 Noon 12th September, 2018 for all submission.
5. OPENING OF BID
The Technical bids will be opened immediately after the deadline for submission 12:00 Noon 12th September, 2018 at the Conference Room on the 3rd Floor of the Ministry in the presence of bidders or their representatives, while the financial bids will be kept un-opened. Note that, the Ministry will not be held liable for misplaced or wrongly submitted Bids.
6. PLEASE NOTE THAT
a. Bids must be in English language and signed by an official authorized by the bidder.
b. Bid submitted after the deadline for submission would be returned un-opened.
c. Bidders shall not bid for more than two (2) Lots.
d. All costs will be borne by the bidders.
e. Only Technically qualified bidders for goods will be contacted at a later date for financial bid opening, while financial bids of un-successful bidders will be returned un-opened.
f. The Federal Ministry of Health reserve the right to annul the process at any time without incurring any liabilities in accordance with Section 28 of the Public Procurement Act 2007.
g. This advertisement shall not be construed as a commitment on the part of the Ministry nor shall it entitle tenderers to make any claims or seek any indemnity from the Ministry by virtue of his/her response to the Advert.
